Tell A Friend Save To Job Cart Apply Online Print This Page   Job Title: Home Training RN Job Code: 325658 Location: Jacksonville, FL Division: Fresenius Medical Services Job Type: Full-time Posted Date: 09/29/09   Assist Home Training Supervisor or DON, in the identification, evaluation, and training of candidates for the home dialysis program.  Responsible for accurate documentation of information related to the patients training and treatment.  Ensure that the information is documented appropriately in the individual patient record and entered into the Proton System.  Responsible for ongoing monitoring, assessment and intervention for home patients including: home visits, clinical appointments, additional training sessions or reviews, lab studies and annual studies such as x-ray and EKG.  Responsible for compliance with patient supply procedures, appropriate documentation and ensuring maintenance of all durable medical equipment, i.e., preventative maintenance, water analysis.  Perform monthly chart audits and report results to supervisor.  Monitor the Peritonitis Rate and PET Test results.  Report the outcomes to the attending physician(s) and the Quality Assurance Committee.  Under the direction of the Home Training Supervisor or DON responsible for making changes in patient treatment plans in conjunction with the physician.  Responsible for scheduling back-up treatment when necessary in Requirements Graduate of an accredited School of Nursing (R.N.). Current appropriate state licensure. Minimum of one-year Hemodialysis experience. Peritoneal dialysis experience preferred.
